Liability Waiver and Assumption of Risk
By checking this box, I acknowledge that I am voluntarily participating in the Synergy Wellness & Physical Therapy 5K. I understand that participation in this event involves inherent risks, including but not limited to falls, contact with other participants, weather conditions, traffic, and other hazards that may result in personal injury, illness, disability, or property damage.
I certify that I am physically able to participate and assume all risks associated with my participation. I hereby release, waive, and discharge Synergy Wellness & Physical Therapy, its owners, employees, volunteers, sponsors, and event partners from any and all claims, liabilities, demands, damages, or causes of action arising out of or related to my participation in this event, including those resulting from ordinary negligence, to the fullest extent permitted by law.
I understand that I am responsible for my own medical expenses and authorize emergency medical treatment if deemed necessary.
If registering a participant under 18 years of age, I certify that I am the parent or legal guardian of the participant and agree to this waiver on their behalf.
